- Your Name: The document must clearly show your (or a household member’s) full name.
- Program Name: The name of the specific qualifying assistance program (e.g., Medicaid, SNAP, Supplemental Security Income (SSI), etc.).
- Issuing Agency: The name of the government agency that issued the award letter or benefit statement.
- Date: The letter must be recent, typically issued within the last 12 months, or have an expiration date in the future to confirm current enrollment in the program.
- Eligibility Confirmation: The document should clearly state that you (or a household member) are eligible for or receiving benefits from the program.
Make sure that all required information is clearly visible and legible on the award letter to avoid delays in Lifeline eligibility verification.

Income-Based Eligibility: Your household income must be at or below 135% of the federal poverty guidelines.
To prove eligibility for Lifeline based on income, you must provide documentation showing your household income is at or below 135% of the federal poverty guidelines. Acceptable documents for income proof include:
- Paycheck Stubs (recent, typically from the past three months)
- Tax Return (most recent federal, state, or Tribal tax return)
- Social Security Benefits Statement (SSA-1099 or equivalent)
- Veterans Administration Benefits Statement
- Pension or Retirement Benefits Statement
- Unemployment or Workers’ Compensation Statement of Benefits
- Divorce Decree or Child Support Award (only if it shows income)
- Letter from an Employer stating your annual income
The documentation must clearly show your income, and you may be required to provide multiple documents if necessary to show a complete picture of your household's income.
